Conclusions The replacement of trans fatty acids with unsaturated fatty acids from unhydrogenated oils nutrition is the single most effective measure for improving blood lipid profiles. Even small amounts of unsaturated fatty acids have a major effect on the ratio of total to HDL cholesterol. The efficacy of replacing SFAs with carbohydrates depends on the effects nutrition on body weight in the long term, nutrition and that effect is uncertain. Our results emphasize the risk of relying on cholesterol alone as a marker of CAD risk. Replacement of carbohydrates with tropical oils markedly raises total cholesterol, which is unfavorable, but the picture changes if effects on HDL and apo B are taken into account. The picture may change again once we know how to interpret the effects of diet on postprandial lipemia, thrombogenic factors, and other, newer markers.
